My Journey with Direct Anterior Hip Replacement


I was first introduced to Direct Anterior Hip Replacement during my residency. My mentor, who was a joint specialist, had just started to learn this technique, and I was lucky enough to learn it alongside him. Research suggests that it takes anywhere between 50 to 100 cases to master this procedure, so it was a valuable experience to learn it without risking any patient complications.


For the five years that I was in residency, I performed both anterior and posterior hip replacements, but I quickly realized that I enjoyed the anterior replacements more. I noticed that my patients had better outcomes, less pain, and a quicker recovery compared to the posterior approach. This is why Direct Anterior Hip Replacement remains my favorite surgery to perform.


When I went to fellowship, I was determined to continue honing my skills in this technique. I did not want to spend a year in advanced training without continuing to perform anterior replacements, so I made sure to choose a fellowship program that offered this training.

The Direct Anterior Hip Replacement offers numerous benefits to patients. Firstly, it is a muscle-sparing technique, meaning that we do not have to cut any muscles. We simply move the muscles to the side and go in between them, resulting in less muscle damage, less inflammation, and less pain for the patient.


Another major benefit is that patients do not have to worry about any leg position precautions post-surgery. With most hip replacements, there are certain positions or activities that patients have to avoid to prevent dislocating the hip. However, with Direct Anterior Hip Replacement, there are basically no positions to avoid.


With this technique, patients are able to sleep, sit, cross their legs, and tie their shoes however they like for the rest of their life. This is a huge benefit that many of my patients appreciate.


Finally, the positioning of the components during Direct Anterior Hip Replacement is a significant advantage. During the surgery, I am able to lie the patient in a supine position and use an X-ray machine to confirm the accuracy and precision of the components. This allows me to ensure that the patient's leg length and offset have been restored correctly, preventing any leg length discrepancies post-surgery.
